Intake Forms

After completing your introductory paperwork, each patient will receive a detailed examination from the doctor in order to determine how we can help and the best method of treatment for your individual needs. The exam will evaluate your musculo-skeletal system, nervous system, balance, posture, and all around health & wellness.

Then, Dr. Cruz will explain your exam findings and discuss your treatment options. Together you will set up health goals and a treatment plan that is both affordable and can easily fit into anyone’s busy schedule.
